About the Opportunity:
We are seeking a skilled and dedicated Mental Health Professional to join our team at the Community Based Outpatient Clinic (CBOC) in McMinnville, TN. As a Mental Health Licensed Clinical Social Worker, you will work closely with the core PACT Team (Primary Care Provider, RN, and Medical Assistant) to deliver comprehensive patient care services.
About the Role:
As a key member of our team, you will be responsible for providing prescribed medical treatment and personal care services to patients with diseases and injuries seeking treatment in the clinic. You will also collaborate with the expanded PACT Team, including family/caregiver, VA, and community-based services involved in developing the patient care plan.
Key Responsibilities:
- Develop and implement individualized treatment plans to meet the unique needs of each patient.
- Provide case management services to Veterans and families as needed throughout the continuum of care.
- Conduct individual counseling/therapy sessions to address social and emotional challenges.
- Lead group therapy sessions to promote social skills and community responsibility.
- Collaborate with the core PACT Team to ensure seamless delivery of patient care services.
- Maintain accurate records of patient interactions and progress.
- Participate in ongoing training and professional development to stay up-to-date on best practices.
- Adhere to all applicable laws, regulations, and organizational policies.
Requirements:
- Master's degree in Social Work from a program registered by the Department of Education.
- Valid, unrestricted LCSW/MSW license in the state of desired employment or any U.S. state or territory, depending on VA contractual requirements.
- Minimum of 1-3 years' post-master direct counseling experience with adult patients.
- Strong communication and interpersonal skills.
- Able to work effectively in a fast-paced environment.
- Proficient in electronic medical record systems.
Salary Information:
The estimated salary for this position is approximately $90,000 - $115,000 per year, based on industry standards and market conditions in the McMinnville, TN area.
