About the Role

CareOregon is seeking a skilled Licensed Practical Nurse to join our team as an Intake and Triage Specialist. In this role, you will be responsible for providing clinical triage support to patients, families, and caregivers, as well as supporting the hospice field staff and informing the clinical staff if a home visit is needed.

Key Responsibilities
  • Manage all incoming phone calls for hospice and provide telephonic triage support to patients, families, and caregivers as needed.
  • Review all incoming referrals and collaborate with clinicians to gather necessary information to determine appropriateness for hospice services.
  • Collaborate with RN and other members of the hospice office regarding referrals to ensure timely intakes are scheduled.
  • Assist in reviewing and assigning RN case manager and Hospice team to new referrals.
  • Make patient visits as needed for back-up and support.
  • Gather and review medical records and medication lists for accuracy.
  • Responsible for documentation in the electronic medical records, assuring accuracy, completeness, and compliance with regulations, certification standards, legal, and ethical standards according to policies and procedures.
  • Assist with hospice chart reviews, as requested.
  • Order DME, medications, or supply refills through PBM/3rd party vendor, as appropriate and in line with the practice standard.
Requirements
  • Graduate of an accredited LPN training program.
  • Current unencumbered LPN license in the State of Oregon.
  • Minimum 1 year experience in hospice, palliative care/oncology, geriatrics, and/or home health.
  • Experience providing triage support for patients.
  • CPR certification at hire or within 6 months in position.
Preferred Qualifications
  • Minimum 2 years' experience in hospice, palliative care/oncology, geriatrics, and/or home health.

Member/Patient Facing: Telephonic, In Person

Hazards: May include, but not limited to, physical, ergonomic, and biological hazards.

Equipment: General office equipment and/or mobile technology

Travel: May include occasional required or optional travel outside of the workplace

Schedule: Occasionally required to work more than 8 hours per day.
