We are seeking a skilled LPN Supervisor to join our team at Ontario Center for Rehabilitation and Healthcare. As a key member of our nursing staff, you will be responsible for overseeing the daily operations of our facility and ensuring the highest level of care for our residents.
Key Responsibilities:- Conduct regular facility rounds to monitor resident care and identify areas for improvement
- Supervise the assignments of aides to ensure proper workflow and efficient use of resources
- Provide guidance and support to staff members during medication assistance times
- Intervene in resident conflicts and behavior issues in a timely and effective manner
- Facilitate communication among staff members to ensure that medical and mental health situations are addressed promptly
- Ensure that all aspects of facility incidents and accidents are completed and documented
- Attend to late admissions and re-admissions, including paperwork and medication changes
- Communicate shift activity to the Enriched Housing Program Coordinator/Administrator via 24-hour report
- Valid state LPN license
- Ability to work effectively as a team member
- Good standing with the State Registry
