About the LPN Supervisor Role

We are seeking a skilled LPN Supervisor to join our team at Ontario Center for Rehabilitation and Healthcare. As a key member of our nursing staff, you will be responsible for overseeing the daily operations of our facility and ensuring the highest level of care for our residents.

Key Responsibilities:
  • Conduct regular facility rounds to monitor resident care and identify areas for improvement
  • Supervise the assignments of aides to ensure proper workflow and efficient use of resources
  • Provide guidance and support to staff members during medication assistance times
  • Intervene in resident conflicts and behavior issues in a timely and effective manner
  • Facilitate communication among staff members to ensure that medical and mental health situations are addressed promptly
  • Ensure that all aspects of facility incidents and accidents are completed and documented
  • Attend to late admissions and re-admissions, including paperwork and medication changes
  • Communicate shift activity to the Enriched Housing Program Coordinator/Administrator via 24-hour report
Requirements:
  • Valid state LPN license
  • Ability to work effectively as a team member
  • Good standing with the State Registry
