Job Summary

Elevate Community Services Inc is seeking a skilled Housing Case Manager to join our team. As a Housing Case Manager, you will play a vital role in providing case management and navigation to individuals residing in our emergency shelter, Travel Inn.

Key Responsibilities
  • Coordinate services for clients, including document collection, assessments, and reporting on client progress.
  • Understand and support clients through the Coordinated Entry System (CES) and identify barriers to housing, such as poor credit, criminal justice involvement, and pending evictions.
  • Guide clients through the housing search and placement process, including document collection, completing assessments, and filling out applications.
  • Confirm client eligibility for programs and services and locate housing opportunities in the community.
  • Encourage clients to proactively search for housing through motivational interviewing and trauma-informed care.
  • Maintain client files and complete documentation within the required timeframe.
  • Collect program data, including tracking housing placements and case notes in HMIS projects.
  • Develop and maintain working knowledge of community resources and act as liaison.
  • Maintain relationships with referring and supporting agencies and represent the agency at community meetings.
  • Advocate for clients' voice and choice in the housing process.
Requirements
  • Bachelor's Degree in social services or a closely related field, preferred.
  • 3 years' experience in housing-related social services.
  • Good documentation skills and excellent communication skills (written and oral).
  • Ability to work well in a team environment and adhere to the highest standard of ethical conduct.
  • Culturally and linguistically sensitive to populations served.
  • Clean driving record and valid driver's license.
