Position Overview
Key Responsibilities:
- 1. Conducts comprehensive physical therapy evaluations, assessing functional limitations, mobility, endurance, and activities of daily living (ADL).
- 2. Recognizes the needs of patients and their families for additional home health services.
- 3. Delivers treatments as prescribed by the physician, adhering to the established care plan.
- 4. Implements suitable therapeutic interventions.
- 5. Maintains detailed records of evaluations, treatments, and patient responses on a visit-by-visit basis, including discharge planning.
- 6. Engages in collaborative case management meetings with team members to synchronize physical therapy with the overall care strategy.
- 7. Offers guidance and education to patients, their families, and other healthcare professionals.
- Bachelor's degree in Physical Therapy or related field.
- Minimum of one (1) year of relevant experience.
- Current licensure as a Physical Therapist.
Work Type: Per Diem
Compensation:
• $65.00 per visit, $90.00 for evaluation visits.
