Licensed Practical Nurse

We are seeking a skilled Licensed Practical Nurse to join our team at the Long Island State Veterans Home. As a critical member of our healthcare team, you will provide high-quality nursing care to our residents, working closely with other professionals to ensure timely and coordinated care.
Key Responsibilities- Provide and ensure nursing care to residents on the nursing unit within the framework of the medical and nursing care plan.
- Monitor residents closely to identify responses to treatments, medication, and/or changes in health status.
- Prepare and dispense medication to residents according to the medical plan of care and consistent with the policies of the Department.
- Directly assist with residents' activities of daily living, including direct care, observation of status and changes, and supervision of non-professional staff.
- Document daily in residents' progress notes all pertinent data, information, and observations, and assist with weekly, monthly, and annual documentation descriptive of resident status and care.
- Communicate immediately all changes and/or incidents to his/her immediate supervisor and complete required documentation.
- Maintain policies and procedures relevant to medications, including management of controlled substances.
- Communicate to supervisor, nursing staff, and other departments and disciplines all pertinent information and changes impacting resident care.
- Listen and respond appropriately to concerns or issues expressed by residents and families.
- Ensure and provide direct care to residents that is consistent with the comprehensive assessment, request of the resident, and the goals of the plan of care.
- Work closely with other professional and disciplines to ensure timely and coordinated care.
- Participate in the Quality Assurance Program of the Department and serve on committees pertinent to the quality of care of the residents.
- Participate in the orientation of new employees to the nursing unit and the clinical in-service of non-professional staff.
- NYS LPN License.
- One year of healthcare experience working with an adult or aging population in a clinical setting.
- One year of long-term care experience.
- Experience using EMR.
Working conditions may involve exposure to infectious disease, blood, body fluids, and/or tissue as well as other unpleasant elements such as accidents, injuries, and illness. May be exposed to and be required to handle varying unpredictable, emergency, and crisis situations. Occasionally subjected to irregular hours and pressure due to multiple calls and inquiries.
Special NotesThe Long Island State Veterans Home is a tobacco-free environment. Tobacco products are strictly prohibited anywhere on our 25-acre campus, including parking lots and outdoor areas on the premises.
This position may require the wearing of respiratory protection, which may prohibit the wearing of facial hair.
This position has been designated as an essential position based on the duties of the job and the functions performed. This means that when the Long Island State Veterans Home is faced with an emergency, employees in such positions are required to remain at their work location or to report to work.
The LISVH is a 24/7 skilled nursing facility. As such, all positions are subject to changes in pass days and shifts as necessary to meet the operating needs of the facility.
The use of Vacation or Personal Leave accruals during the first 6 months of employment will not be authorized unless prior approval has been granted by Human Resources.
The selected candidate must successfully clear a background investigation. Prior to start date, the selected candidate must meet the following requirements: Successfully complete pre-employment physical examination and obtain medical clearance from the Long Island State Veterans Home Employee Health Department, clear a background investigation, submit three (3) written references, and provide a copy of the required license(s)/certificate(s).
In accordance with federal and State regulations, that all hospital and nursing homes require personnel to be vaccinated against COVID-19, Candidates who are not already fully vaccinated or partially vaccinated must obtain the first dose of a COVID-19 vaccine within three (3) calendar days of acceptance of a conditional job offer and must obtain any subsequent doses in accordance with that particular vaccine manufacturer's protocol. Candidates who are partially vaccinated, but not yet fully vaccinated, must complete their vaccination series within three (3) calendar days of job offer or in accordance with that particular vaccine manufacturer's protocol, whichever comes later.
The State regulation also includes those who may be affiliated with or interact with employees of a hospital or nursing home. The regulations allow for limited exemptions with reasonable accommodations, consistent with applicable law.
