Job Summary:

At Stony Brook Medicine, we are seeking a highly skilled Licensed Practical Nurse to join our team at the Long Island State Veterans Home. As a critical member of our healthcare team, you will provide exceptional care to our residents, working closely with other professionals and disciplines to ensure timely and coordinated care.

Key Responsibilities:
  • Provide and ensure nursing care to residents on the nursing unit within the framework of the medical and nursing care plan.
  • Monitor residents closely to identify responses to treatments, medication, and/or changes in health status.
  • Prepare and dispense medication to residents according to the medical plan of care and consistent with the policies of the Department.
  • Directly assist with residents' activities of daily living, including direct care, observation of status and changes, and supervision of non-professional staff.
  • Document daily in the residents' progress notes all pertinent data, information, and observations, and assist with weekly, monthly, and annual documentation descriptive of resident status and care.
Requirements:
  • NYS LPN License.
  • One year of healthcare experience working with an adult or aging population in a clinical setting.
  • One year of long-term care experience.
