**Job Summary:**

We are seeking a skilled Nurse Navigator to join our team at Austin Regional Clinic (ARC). In this role, you will play a vital part in coordinating healthcare services for patients with complex health conditions. As a Nurse Navigator, you will serve as an educator and advocate for patients, working closely with primary care providers to develop personalized plans of care.

The ideal candidate will possess excellent communication and interpersonal skills, with the ability to work effectively in a team environment. You will be responsible for reviewing risk stratification reporting, prioritizing patients for clinical outreach, and developing trusting relationships with patients and their families. Your expertise in case management, home health, or administrative experience will be valuable assets in this role.

**About the Role:**

In this position, you will:

  • Review risk stratification reporting to prioritize patients for clinical outreach
  • Contact targeted patients, develop plans of action, ensure physician's care plan is followed, and monitor status of these plans
  • Attend patient visits with the providers, as needed
  • Develop trusting relationships with patients and their families
  • Work collaboratively with primary care providers and clinic staff
  • Support Medicare Wellness provider staff, as needed

**Requirements:**

To be successful in this role, you must have:

  • A graduate degree in nursing from an accredited school
  • At least one year of experience working in direct patient care in a clinical environment
  • Excellent customer service skills and the ability to work effectively in a team environment
  • Basic Life Support Certification (BLS)

**Estimated Salary:** $65,000 - $85,000 per year, depending on experience
