Key Responsibilities
  • Assess and develop discharge plans for adult and geriatric patients, taking into account their unique needs and circumstances.
  • Collaborate with patients, families, physicians, and other healthcare professionals to ensure comprehensive care and support.
  • Provide emotional, social, and financial support to patients and families, addressing complex family dynamics and psychosocial issues.
  • Mobilize community resources to meet identified needs, promoting patient-centered care and coordination.
  • Document all interventions and services provided, maintaining accurate and timely records.
  • Participate in multidisciplinary rounds and contribute to care coordination, ensuring seamless transitions and optimal patient outcomes.
Requirements
  • Master's Degree in Social Work or Counseling from an accredited institution.
  • Current licensure in the State of Maryland – LMSW/LGPC required; LCSW-C/LCPC preferred.
  • Minimum of 1 year of experience in a healthcare setting, preferably in a hospital environment.
