Job Title: Hospice Home Health RN

VNA Health Group is seeking a compassionate and skilled Registered Nurse to join our team in providing end-of-life care to patients in their homes.

Job Summary:

The Registered Nurse will be responsible for providing comprehensive nursing care to patients with terminal illnesses, including but not limited to:

  • Providing prescribed medical treatment and personal care services to patients in their homes.
  • Utilizing the nursing process to assist in data collection, planning, and evaluation of nursing care.
  • Demonstrating nursing knowledge and skills in the implementation of prevention, therapeutic, and rehabilitative nursing care.
  • Taking and recording patients' vital signs.
  • Administering specified medication, orally or by subcutaneous or inter-muscular injection, and noting time and amount on patients' charts.
  • Reinforcing instructions and explanations of the patient Plan of Care or teaching plan.
  • Using oral and written communication to convey pertinent information to members of the health care team in a timely manner.
  • Reporting any significant changes in the patient's health status to the appropriate individual(s).
  • Maintaining a safe environment for the patient and self.
  • Identifying unmet learning needs and seeking assistance from others in meeting those needs.
  • Complying with company policies and procedures.
  • Completing all required forms, reports, time cards, itineraries, etc, accurately and within designated timeframe.
  • Recognizing and following established lines of communication.
  • Performing other duties as assigned.
Requirements:

Qualified candidates must:

  • Hold a diploma, Associate's or Bachelor's degree in Nursing.
  • Have at least 1 year of hospital experience.
  • Have at least 1 year of Hospice, Home care, and/or Community Health experience.
  • Be able to work effectively with persons and communities from diverse cultural, social, and ethnic backgrounds.
  • Be able to work independently with minimal supervision.
  • Be able to maintain strict confidentiality.
