Job Title: Home Health Registered Nurse

VNA Health Group is seeking a skilled and compassionate Registered Nurse (RN) to join our Home Health team in Cedar Grove, New Jersey.

Job Summary:

We are looking for a dedicated and experienced RN to provide high-quality nursing care to patients in their homes. As a Home Health Registered Nurse, you will work closely with our interdisciplinary team to develop and implement individualized care plans that meet the unique needs of each patient.

Responsibilities:
  • Assess and evaluate patients' physical and emotional needs, and develop and implement individualized care plans.
  • Provide skilled nursing care to patients in their homes, including wound care, medication management, and education on disease management.
  • Collaborate with other healthcare professionals to ensure seamless care coordination and communication.
  • Document all patient interactions and care provided in a timely and accurate manner.
  • Participate in ongoing education and training to stay current on best practices and industry developments.
Requirements:
  • Active New Jersey RN license.
  • Bachelor's degree in Nursing preferred.
  • 1+ year of supervised clinical experience as a registered nurse.
  • Excellent verbal and written communication skills.
  • Strong computer skills.
  • Excellent organizational and interpersonal skills.
