Job Overview

Position Title: Personal Care Assistant

Role Summary:

The Personal Care Assistant provides essential support services under the guidance of a Registered Nurse or Therapist. Assigned to specific clients, this role is crucial in ensuring the comfort and well-being of individuals receiving care.

Reporting Structure: Reports to the Client Services Director or Nursing Supervisor.

Qualifications Required:

  1. Completion of a recognized certification training program, including a written skills assessment and competency evaluation.
  2. Must be at least eighteen (18) years old.
  3. A minimum of six (6) months of experience in a supervised environment, ideally within a healthcare setting.
  4. Ability to read, write, and adhere to a written Plan of Care.
  5. Strong interpersonal communication skills.
  6. Possession of a valid driver's license, a clean driving record, and reliable transportation.

Key Responsibilities:

  1. Assist with basic procedures as directed by the therapist, including:
  • Range of motion exercises
  • Support in ambulation or physical exercises
Provide personal care services, which may include:
  • Bathing
  • Shampooing
  • Skin and nail care
  • Oral hygiene
  • Shaving
  • Dressing assistance
Perform household tasks essential for health care at home, such as:
  • Meal preparation and feeding
  • Laundry
  • Light housekeeping duties
Assist in administering medications that clients typically self-administer, under the supervision of a Registered Nurse, in accordance with state regulations and agency policies. Report any changes in the client's condition or needs to the Registered Nurse. Document care provided and maintain accurate records of services rendered and observations made. Ensure personal safety and a secure environment for clients by adhering to infection control protocols and reporting any unsafe conditions to the supervisor or Case Manager. Utilize equipment safely, ensuring proper training has been received before use, and communicate any educational needs to the supervisor. Engage effectively with all members of the care team through verbal updates and participation in meetings as required. Maintain confidentiality in all job-related matters. Participate in mandatory training programs to meet compliance standards and identify personal learning needs to the agency supervisor. Perform additional duties as necessary to support client care.
