Job Summary

The Housing Support Clinician is a vital member of the Housing Support Program team, providing comprehensive support services to individuals living in permanent subsidized housing. This role involves conducting assessments, developing treatment plans, and providing crisis intervention to promote housing stability and overall well-being.

Key Responsibilities
  • Conduct comprehensive assessments to evaluate clients' mental health status, substance use, and psychosocial needs.
  • Collaborate with clients to create person-centered treatment plans tailored to their housing needs.
  • Provide crisis intervention to assist clients with stabilization and development of coping strategies and life skills.
  • Regularly review and adjust treatment plans based on client progress with maintaining housing stability.
  • Work with housing case managers and other professionals to address clients' holistic needs, including housing stability and healthcare access.
  • Provide care coordination to ensure stability of housing and independent living skills and facilitate referrals and linkages to internal/external community services/providers.
Requirements
  • Master's degree in counseling, social work, or related field.
  • Associate license or eligibility to receive associate license (within 12 months of hire).
  • One year experience providing clinical/case management/community support service or similar.
  • Proficient with technology, electronic health records, and computer applications such as Microsoft Word, Excel, Outlook, and Teams.
Working Conditions

The Housing Support Clinician will work in a community setting, typically Monday-Friday 8:15am-5:00pm, with some evening and weekend hours required. Service delivery will be in-person and via telehealth/telephone. Travel throughout DBHDD Region 3 will be necessary. The ability to use a computer, telephone, fax, copy, and scanning machine is required.
