Role Summary and Requirements

The Rehabilitation Case Coordinator (RCC) is responsible for ensuring high-quality, patient-focused care by managing rehabilitative services to achieve optimal outcomes. The RCC coordinates, directs, and tailors care to meet the unique needs of each rehabilitation patient. Collaboration with the Rehabilitation Program Director and the Facility Case Management Director is essential for effective departmental operations.

Key Responsibilities:

  • Provide orientation to patients, families, and caregivers regarding the rehabilitation process, including the role of the case manager, rehabilitation philosophy, and criteria for continued care and discharge.
  • Act as the primary coordinator for patient, family, and caregiver education, encouraging active participation in discussions about care plans, goals, and progress.
  • Ensure the execution of individualized treatment plans that align with the patient's strengths, needs, and preferences, facilitating their involvement throughout the rehabilitation journey.
  • Document findings from Discharge Planning Evaluations (DPE) and psychosocial assessments, communicating the social, financial, and discharge needs of patients and their families.
  • Promote positive patient interactions that enhance the quality of service provided.
