Case Manager RN

At TriStar Skyline Medical Center, our nurses are the backbone of our healthcare team. We're seeking a skilled Case Management RN to join our family and help us deliver exceptional patient care.
About the RoleThe Case Management RN will play a vital part in promoting patient-centered care by coordinating the plan of care, managing length of stay, and ensuring appropriate resource management. This role requires strong communication and collaboration skills, as well as the ability to think critically and make informed decisions.
Responsibilities- Identify patients at risk for adverse outcomes during transition from one level of care to another.
- Perform comprehensive assessments of psychosocial, medical, and discharge needs of patients and families.
- Develop strategies to mitigate readmission risk, including education on accessing healthcare resources and community-based resources.
- Coordinate the plan of care and drive discharge planning through collaboration with the multidisciplinary healthcare team.
- Partner with Social Services to ensure post-acute medical needs and level of care are appropriate.
- Evaluate progression of care using evidence-based tools and approved criteria.
- Make referrals to third-party payer and disease and case management programs for recurring patients and patients with chronic disease states.
- Facilitate patient throughput with a focus on effective care transition, quality, and efficiency.
- Document professional recommendations, discharge plans, care coordination interventions, and case management activities.
- Act as a liaison through effective and professional communications between and with physicians, patient/family, hospital staff, and outside agencies.
- Direct activities to identify and provide for the needs of the under-resourced patient population.
- Participate in performance improvement activities, including identifying, documenting, and intervening when avoidable days occur.
