Overview: The Transition Navigator plays a crucial role in supporting youth and young adults, particularly those impacted by the foster care system. This position is dedicated to fostering positive change and enhancing the quality of life for individuals aged 14 to 29.

Mission: At The New Foster Care (TNFC), our goal is to implement transformative initiatives within the Michigan Foster Care system. We strive to create a significant and measurable impact on the lives of youth who have navigated the complexities of foster care.

Key Responsibilities:

  • Build trusting relationships with youth and their families to promote empowerment and hope.
  • Conduct assessments to identify strengths and needs, collaborating with clients to develop actionable Future Plans.
  • Provide in-vivo coaching, teaching essential skills in real-world settings.
  • Assist clients in managing high-risk behaviors, including substance abuse and self-harm.
  • Support the development of decision-making skills and enhance overall executive functioning.
  • Deliver trauma-informed care, recognizing the effects of past experiences on current behaviors.
  • Connect clients with community resources and facilitate appropriate referrals.
  • Maintain up-to-date knowledge of eligibility criteria and assist clients with application processes.
  • Utilize core competencies such as Futures Planning and Social Problem Solving in interventions.
  • Document and track progress related to transition domains and outcomes.

Qualifications:

  • Master's degree in social work, psychology, or a related field, or a bachelor's degree with relevant experience.
  • Experience working with youth and young adults in foster care is preferred.
  • Strong communication skills, both verbal and written, with the ability to engage effectively with diverse populations.
  • Proficient in documentation and case management practices.
  • Ability to work independently and manage a flexible schedule to meet client needs.

  • Valid driver's license and reliable transportation.
  • Commitment to maintaining professional standards and ethical guidelines.
  • Willingness to participate in ongoing training and professional development.

This position is essential in guiding youth through their transition journey, ensuring they have the necessary support and resources to thrive.
