Social Services Manager Job Description

We are seeking a highly skilled Social Services Manager to join our team at AndHealth. As a Social Services Manager, you will play a crucial role in supporting patients by connecting them to local services and resources.

Key Responsibilities:
  • Patient Assessment: Conduct comprehensive assessments of patients' social, emotional, and environmental needs to identify areas for support and intervention.
  • Service Coordination: Identify and connect patients with appropriate local services, including housing, food options, behavioral health resources, transportation, and more.
  • Medicaid Enrollment: Assist patients in understanding and completing the Medicaid enrollment process to ensure access to essential healthcare services.
  • Resource Management: Maintain an up-to-date directory of community resources and services available to patients.
  • Collaboration: Work closely with healthcare providers, case managers, and other social service organizations to coordinate care and ensure a seamless approach to patient support.
  • Advocacy: Advocate on behalf of patients to ensure they receive the services and support they need.
  • Documentation: Accurately document all patient interactions, assessments, and service referrals in the AndHealth app and potentially the EHR.
  • Follow-Up: Provide ongoing follow-up with patients to ensure they are successfully connected to services and support their continuous engagement with needed resources.
Requirements:
  • Bachelor's or Master's degree from an accredited institution.
  • A CCM is preferred.
  • An LSW license in the state of Ohio is preferred.
Preferred Qualifications:
  • Minimum of 2 years of experience as a social worker, preferably in a healthcare or community services setting in and around Columbus.
  • Familiarity with local community resources and services and understanding of Medicaid enrollment processes.
  • Excellent interpersonal, communication, and organizational skills. Ability to work independently and as part of a multidisciplinary team.
  • Proficiency in using electronic health record (EHR) systems and adapting to new technologies as needed.

Additional Requirements:
  • (PPD) TB Skin Test - Proof of negative TB skin test within the last 12 months.
  • (MMR) Measles, Mumps and Rubella or A Blood Titer proving immunity.
  • Varicella - (2) documented doses or A Blood Titer proving.
  • Hep B3 Series.
  • (Flu) Influenza required.
  • COVID Vaccine required.
