Social Services Director

We are seeking a highly skilled Social Services Manager to join our team at AndHealth. As a Social Services Manager, you will play a crucial role in supporting patients by connecting them to local services and resources.
Key Responsibilities:- Patient Assessment: Conduct comprehensive assessments of patients' social, emotional, and environmental needs to identify areas for support and intervention.
- Service Coordination: Identify and connect patients with appropriate local services, including housing, food options, behavioral health resources, transportation, and more.
- Medicaid Enrollment: Assist patients in understanding and completing the Medicaid enrollment process to ensure access to essential healthcare services.
- Resource Management: Maintain an up-to-date directory of community resources and services available to patients.
- Collaboration: Work closely with healthcare providers, case managers, and other social service organizations to coordinate care and ensure a seamless approach to patient support.
- Advocacy: Advocate on behalf of patients to ensure they receive the services and support they need.
- Documentation: Accurately document all patient interactions, assessments, and service referrals in the AndHealth app and potentially the EHR.
- Follow-Up: Provide ongoing follow-up with patients to ensure they are successfully connected to services and support their continuous engagement with needed resources.
- Bachelor's or Master's degree from an accredited institution.
- A CCM is preferred.
- An LSW license in the state of Ohio is preferred.
- Minimum of 2 years of experience as a social worker, preferably in a healthcare or community services setting in and around Columbus.
- Familiarity with local community resources and services and understanding of Medicaid enrollment processes.
- Excellent interpersonal, communication, and organizational skills. Ability to work independently and as part of a multidisciplinary team.
- Proficiency in using electronic health record (EHR) systems and adapting to new technologies as needed.

- (PPD) TB Skin Test - Proof of negative TB skin test within the last 12 months.
- (MMR) Measles, Mumps and Rubella or A Blood Titer proving immunity.
- Varicella - (2) documented doses or A Blood Titer proving.
- Hep B3 Series.
- (Flu) Influenza required.
- COVID Vaccine required.
- Additional health requirements may also be required.
