Kaiser Permanente is seeking a skilled Social Work Professional to join our team in McLean, Virginia.
Job DescriptionWe are looking for a Licensed Clinical Social Worker to collaborate with our healthcare team to provide high-quality care to our members. As a key member of our team, you will be responsible for coordinating appropriate services, promoting optimal levels of independence, and enhancing the quality of life for our patients.
Key Responsibilities- Conduct comprehensive psychosocial assessments to evaluate patient goals, social support systems, resources, health status, functional limitations, psychological status, environmental factors, and response to treatment.
- Develop and monitor a plan of care designed to promote the patient's optimal level of functioning and enhance the quality of life.
- Identify, facilitate, and advocate for appropriate organizational and community resources to meet the plan of care.
- Ensure consistent and reliable documentation of case management activities in compliance with all organization and department standards.
- Analyze patient and program outcomes to identify improvements in program, quality, and cost effectiveness of case management activities.
- Minimum one (1) year of clinical social work experience in a healthcare setting required.
- Masters degree in social work (MSW) required.
- Licensed Independent Social Worker (District of Columbia) within 6 months of hire OR Licensed Graduate Social Worker (District of Columbia) within 6 months of hire.
- Licensed Master Social Worker (Maryland) within 6 months of hire.
- Licensed Master's Social Worker (Virginia) within 6 months of hire.
