Job Summary

Kaiser Permanente is seeking a skilled Social Work Licensed Clinical Social Worker to join our team in Arlington, Virginia.

Key Responsibilities
  • Collaborate with healthcare teams to coordinate patient care and services.
  • Conduct comprehensive psychosocial assessments to identify patient needs and develop personalized plans of care.
  • Facilitate access to community resources and services to support patient well-being.
  • Provide supportive counseling and education to patients, families, and caregivers.
  • Monitor and analyze patient outcomes to improve program effectiveness.
Requirements
  • Master's degree in social work (MSW) required.
  • Minimum three years of clinical experience and two years in case management.
  • Licensed Clinical Social Worker (LCSSW) certification in Maryland, Virginia, and the District of Columbia within six months of hire.
  • Case Manager Certificate within 36 months of hire.
Preferred Qualifications
  • Experience with computer software programs in a Windows environment.
  • Knowledge of community systems and resources in the defined service area.
  • Knowledge of regulatory issues for the Mid-Atlantic area.
