JOB OVERVIEW

The Nurse Practitioner will serve as a key clinical provider within the Good Shepherd Rehabilitation Network, overseeing patient care from admission through transfer while embodying the organization's mission. This role involves collaboration with the medical team on the TBI unit, including daily patient assessments and providing support during outpatient appointments.

KEY RESPONSIBILITIES

  • ENSURE FINANCIAL VIABILITY
  • The Nurse Practitioner will address all patient needs, facilitating extended operational hours to enhance revenue, improve patient satisfaction, and provide flexibility for high-acuity patient admissions.
  • ACHIEVE SPECIFIC CLINICAL OUTCOMES
  • Conduct daily evaluations of assigned inpatients to assess their condition, prescribe treatments, plan care, coordinate services, implement interventions, and evaluate outcomes.
  • Perform comprehensive clinical assessments for necessary interventions.
  • Conduct initial evaluations, including history and physical examinations for all assigned patients to facilitate the admission process.
  • Order and interpret relevant diagnostic and laboratory tests.
  • Execute approved therapeutic or diagnostic procedures based on the clinical status of patients, ensuring proper documentation.
  • Facilitate a seamless transition to aftercare by engaging with families and treating physicians as needed.
  • Collaborate with the case management team to develop care and discharge plans.
  • Utilize prescriptive authority as permitted by regulatory boards.
  • Document all prescribed medications in the patient's medical record.
  • Monitor patients for changes in condition and implement appropriate interventions to stabilize them.
  • Provide family-centered care by communicating effectively with families regarding their loved ones' health needs.
  • Respond to clinical concerns raised by nursing staff and therapists, developing a clinical plan and consulting with physicians as necessary.
  • Act as a clinical resource and leadership consultant for nursing staff and team members.
  • Maintain open lines of communication with referring and consulting physicians to address any barriers to patient progress.
  • Engage in compliance, accreditation, and educational initiatives for patients, families, and staff, providing both formal and informal education to enhance nursing skills and critical thinking.
  • Promote awareness of subtle clinical changes that may require immediate assessment.
  • Exemplify advanced professional nursing through conduct, communication, respect, leadership, ethical decision-making, critical thinking, and problem-solving.
  • Demonstrate commitment to professional development through active participation in relevant organizations and continuing education.
  • Contribute to the nursing knowledge base through sharing ideas and research in professional settings.
  • Collaborate with hospital administration to identify and address organizational needs.
  • ENSURE QUALITY SERVICE

  • For inpatient, outpatient, and consulting services as required.
  • PARTICIPATE IN ON-CALL SCHEDULE
  • By being available for inpatient and outpatient coverage as needed.

QUALIFICATIONS

  • Education
  • Master's Degree in Nursing is required.
  • Graduation from an accredited Nurse Practitioner program is required.
  • Work Experience
  • 1-2 years of experience in Critical Care is required.
  • 1-2 years of Nurse Practitioner experience is preferred.
  • Licenses / Certifications
  • Certification as a Nurse Practitioner is required.
