Job Title: LPN Charge Nurse

The LPN Charge Nurse plays a vital role in providing high-quality care to individuals with complex medical needs. As a skilled nurse leader, you will be responsible for coordinating care with the LPN team, physicians, and other healthcare providers to ensure seamless delivery of services.

Key Responsibilities:
  • Develop and implement individualized care plans to meet the unique needs of each client
  • Assess and identify potential health problems, reporting changes in condition to ensure timely interventions
  • Manage and maintain electronic documentation of health status, including medication administration and G/J tube feeding
  • Collaborate with other providers to ensure comprehensive care, including staff LPNs, physicians, and community healthcare providers
  • Advocate for the individual as necessary, serving as a liaison between family, Georgia Options, and other stakeholders
  • Assist Direct Support Professionals in providing community living support and non-skilled personal care

Requirements:

  • Current LPN licensure in the state of Georgia
  • Experience in skilled nursing care, preferably in a home setting
  • Strong communication and interpersonal skills
  • Ability to work independently and as part of a team
