We are seeking a compassionate and skilled Hospice Spiritual Care Counselor to join our team at Residential Home Health and Hospice. As a key member of our interdisciplinary team, you will play a vital role in providing spiritual support to our patients and their families. Our company is grounded in the belief that outstanding care is best delivered in a team-based environment, and we are committed to supporting the spiritual needs of our patients.

The successful candidate will have a graduate degree from an accredited seminary or school of theology (BD, MDiv or equivalent theological degree) and experience working with death and dying, individuals/family/caregiver. They will be able to facilitate the development of individualized plans of care, provide direct spiritual care to patients and families/caregivers, and serve as liaison and support to community chaplains and spiritual care counselors. In addition, they will provide consultation, education, and support to the interdisciplinary group on spiritual care. The estimated salary for this position is between $65,000 and $80,000 per year.

What You'll Do in this Role
In this role, you will ensure spiritual assessment of patients and families/caregivers in the hospice program and that appropriate services are rendered in a timely manner. You will also facilitate the development of individualized plans of care by participating in comprehensive assessments to meet identified spiritual needs. Furthermore, you will provide direct spiritual care to patients and families/caregivers, serve as liaison and support to community chaplains and spiritual care counselors, and provide consultation, education, and support to the interdisciplinary group on spiritual care. The successful candidate will be a compassionate and skilled Hospice Spiritual Care Counselor who can work effectively in a team-based environment.
