  • Full-Time or Part-Time

COMPENSATION:

  • $125,000 to $155,000 Annual Income
- $105,000 to $125,000 Base Salary
- $20,000 to $30,000 per Year in RVU Bonuses

This practice provides care to patients in skilled nursing facilities (SNFs), assisted living facilities (ALFs), independent living facilities (ILFs), and private residences.

They are currently expanding their team and will collaborate with providers during the interview process to establish a mutually-agreeable geographic territory for patient care.

This organization is committed to delivering accessible, high-quality primary care to homebound and facility-based geriatric patients in the comfort of their own environments.

They currently have a dedicated team of over 15 physicians and nurse practitioners who ensure that patients receive exceptional medical care without the difficulties associated with traveling to a provider's office.

Their services encompass primary care, chronic care, psychiatric care, post-acute care, transitional care, and remote patient monitoring.

POSITION DESCRIPTION:
The nurse practitioner will act as the primary care provider for a dedicated panel of geriatric patients.

All patients are located in facilities or private homes.

Depending on the territory, the practice can offer various combinations of facilities (ALFs / ILFs / SNFs) and home-based patients.

During the initial phone interview, the practice will assess the provider's location and discuss this in detail.


In SNF settings (with no travel during the day), providers typically see an average of 15 to 16 patients per day.


When traveling between ALFs, ILFs, and homes, providers see an average of 9 to 12 patients per day (this is adjusted based on actual travel time).

Specific responsibilities of the role include, but are not limited to:

  • Conduct health assessments to identify patient needs
- Develop personalized care plans
- Regularly evaluate patient conditions and adjust treatment plans as necessary
- Administer medications, treatments, and other therapeutic interventions as prescribed
- Perform minor procedures and wound care as required (and as settings permit)
- Monitor vital signs and patient responses to treatment
- Educate patients and families on health management, disease prevention, and treatments
- Provide guidance on lifestyle modifications and self-care practices
- Collaborate with physicians, nurses, and other professionals to coordinate comprehensive care
- Ensure compliance with all relevant healthcare regulations and standards

The practice utilizes the eClinicalWorks EHR.

On-call responsibilities involve phone triage only with very low volume. Providers rotate the on-call duty, with a shift every 10 weeks or less for full-time providers. Part-time providers will participate in on-call on a negotiated schedule (if at all).

REQUIREMENTS:

  • Ohio NP license
  • DEA
  • 1+ year advanced practice experience
- Previous experience in geriatrics, LTC, SNFs, house calls, etc.
