Job Title: LPN - Home Health Nurse

We are seeking a skilled and compassionate LPN to join our team at All Ways Caring HomeCare. As a Home Health Nurse, you will provide high-quality care and support to clients in their homes, helping them to maintain their independence and dignity.

Job Summary:
  • Provide direct and indirect care to clients in their home environment
  • Instruct, motivate, and assist clients in health maintenance and disease prevention
  • Direct the efforts of the Caregiver when involved in the client's plan of care
  • Perform administrative and/or field/clinical responsibilities while receiving oversight and clinical direction from a Registered Nurse
Responsibilities:
  1. Provides skilled nursing care/treatment to clients per Physicians orders and Plan of care
  2. Accurately documents nursing actions of all care given, individual's health status, and communicate with family and physicians
  3. Documents the progress and outcomes for established goals and informs physician, case manager, and families of changes in client's medical condition and needs
  4. Provides skilled nursing treatments per physician's orders and within the scope of practice
  5. Medication administration and management as needed
  6. Provides health teaching of client and assisting client in learning appropriate self-care techniques and safety measures
  7. Conducts nursing assessment visit as needed
  8. Reports all incidents per operation procedure and regulatory standards
  9. Follows policies and procedures according to operation, state and regulatory standards
  10. Uses safe body mechanics to prevent personal injury to self or client
  11. Maintains confidentiality of patient related information and honor patient rights
  12. Consults, seeks advice, and follows direction of the RN in fulfillment of care of the client
  13. Completes Start of Care assessments in person, where applicable and in accordance with state regulations (as applicable)
  14. Completes supervisory visits based on compliance and regulations to include review of plan of care, caregiver competency assessment, and completion of required documentation
  15. Works closely with Director or Nursing/Nurse Supervisor and/or Branch Manager in planning, implementing, and evaluating programs and services to address the health needs of persons served
  16. Attends and participates in staff meeting, in services, and other meetings as requested
Requirements:
  • Degree in Nursing from an accredited school
  • Experience in administering medications
  • One or more years of nursing experience preferred
  • Home health care experience preferred
  • Current and unrestricted license in the state of the operation
  • CPR professional certification
  • Basic clinical skills
  • Professional appearance and demeanor
  • General knowledge of computers and the ability to learn industry specific software and documentation applications
  • Excellent oral, written, and interpersonal communication skills with all levels of personnel, internal and external to the company
  • Excellent organizational and time management skills with a keen attention to details
  • Capable of working responsibly with highly confidential information
  • Ability to work effectively independently or in a team environment
  • Ability to develop critical thinking skills, research situations, solve complex problems and deal with a variety of issues
  • Ability to learn and complete various educational and position specific goals
