Key Responsibilities:

  • Establish and maintain effective relationships with patients, caregivers, and healthcare providers.
  • Perform comprehensive assessments, develop care plans, and provide ongoing care to patients.
  • Consult with Physician Leaders and coordinate with Primary Care Physicians as needed.
  • Participate in quality initiatives, including HEDIS measure campaigns and clinical after-hours on-call programs.

Requirements:

  • Certification through the American Academy of Nurse Practitioners (AANP), American Nurses Credentialing Center (ANCC), or the National Commission on Certification of Physician Assistants (NCCPA).
  • Current, unrestricted NP or PA licensure in the State of Practice.
  • Access to reliable transportation.

Preferred Qualifications:

  • 2+ years of clinical experience in primary care, home-based medical care, geriatrics, palliative care, emergency/urgent care, or similar settings.
  • Proven knowledge of long-term care, community resources, and cost-effective alternatives regarding patient care delivery systems.
  • Proven knowledge of CPT, ICD-10, and HCPC codes.
  • Proven knowledge of clinical standards of care.
  • Demonstrated awareness of NCQA requirements, CMS guidelines, Medicaid/Medicare contracts, and benefit systems.
