About the Role

We are seeking a highly skilled and compassionate MDS Coordinator to join our team at Trilogy Health Services. As a key member of our care team, you will be responsible for assessing the needs of our residents and developing personalized care plans to ensure their optimal health and well-being.

Key Responsibilities
  • Manage the Resident Assessment Instrument (RAI) process to assess residents' individual needs and create unique care plans based on your findings.
  • Promote cross-functional communication to ensure care plans are followed and residents receive high-quality care.
  • Ensure accurate and timely reimbursement for resident care services.
  • Stay up-to-date with current federal, state, and local standards, guidelines, and regulations.
Requirements
  • Current, valid state RN license.
  • At least 2 years of experience in MDS (resident assessment) and prior experience in Long-Term Care.
