Job Summary

Humboldt Park Health is seeking a skilled Behavioral Health Therapist LCSW or LCPC to join our team. As a key member of our clinical team, you will provide individual and group therapy to patients using evidence-based modalities.

Key Responsibilities
  • Provide individual therapy to assigned caseload of patients using evidence-based modalities, including CBT, DBT, ACT, ART, RET, EMDR, and Motivational Enhancement Therapy.
  • Provide group therapy as assigned based on clinical program design.
  • Contribute to the design and continuous improvement of the clinical program.
  • Participate in Interdisciplinary Treatment Team Meetings and collaborate to develop a treatment plan with individualized interventions for each patient.
  • Provide supervision to Licensed Social Workers who are focused on care management and discharge planning.
  • Maintain accurate and appropriate records of therapeutic activities in patient's medical record.
Requirements
  • Licensed clinician LCSW or LCPC.
  • BLS Certified.
  • Most have a Masters Degree in Social Work.
  • Bilingual Spanish/English a plus.
