CenterWell Home Health is seeking a skilled Clinical Manager RN to oversee the delivery of high-quality patient care in a home health setting. As a key member of our team, you will be responsible for coordinating and managing the clinical operations of our home health program, ensuring that patients receive the best possible care and outcomes.
Key Responsibilities- Develop and implement clinical policies and procedures to ensure compliance with regulatory requirements and Company standards.
- Supervise and manage a team of clinical staff, including nurses, therapists, and home health aides, to ensure that patients receive high-quality care and services.
- Conduct regular assessments and evaluations of patient care to identify areas for improvement and implement changes as needed.
- Collaborate with the Branch Director and other stakeholders to develop and implement business strategies and plans to achieve Company goals and objectives.
- Ensure that all clinical staff are properly trained and equipped to provide high-quality patient care and services.
- Monitor and analyze clinical data to identify trends and areas for improvement, and implement changes as needed.
- Develop and maintain relationships with referral sources, including physicians, hospitals, and other healthcare providers, to ensure a steady flow of referrals and business growth.
- Participate in performance improvement activities, including quality improvement initiatives and patient satisfaction surveys.
- Graduate of an accredited School of Nursing.
- Current state license as a Registered Nurse.
- Proof of current CPR.
- Valid driver's license, auto insurance, and reliable transportation.
- Two years of experience as a Registered Nurse, with at least one year of management experience in a home care or hospice setting.
- Experience with Homecare Homebase (HCHB) software and OASIS.
- Strong clinical background in home care operations and quality assurance.
