Key Responsibilities:- Deliver palliative care to patients in various settings, including hospitals, clinics, and patient homes.
- Collaborate with physicians and other healthcare professionals to develop and implement individualized care plans.
- Conduct face-to-face encounters with patients to assess their needs and provide education and support.
- Initiate palliative care consultations and develop treatment plans in accordance with state laws and regulations.
- Communicate effectively with patients, families, and healthcare teams to ensure seamless care coordination.
- Provide training and education to staff members on palliative care best practices.
- Meet productivity standards for patient visits and maintain accurate patient records.
- Current unrestricted license to practice as a registered professional nurse in the state of employment.
- Graduate of an approved Nurse Practitioner program.
- Current unrestricted license to practice as a Nurse Practitioner by the Board of Nursing in the state of employment.
- Current and continued CPR certification.
- Certified Hospice and Palliative Care Nurse (CHPN) preferred.
- Minimum of 2 years of palliative care and/or hospice experience required.
