Patient Care Coordinator

Katy Trail Community Health is seeking a highly skilled Patient Care Coordinator to join our dynamic team. As a Patient Care Coordinator, you will play a vital role in ensuring that patients receive comprehensive medical care coordination tailored to their needs.
Key Responsibilities- Provide planned care for patients who may have been identified by diagnosis or condition, as being a member of a vulnerable population, and/or as needing transition of care from an outside facility.
- Address barriers to care which may affect patient compliance with their care plan, including following up for important appointments.
- Work collaboratively with healthcare providers, addressing inquiries and resolving any issues that may arise.
- Handle a variety of tasks that require organizational skills and attention to detail.
- Strong computer skills and proficiency in word processing, enabling you to manage patient records efficiently.
- Ability to organize and categorize information effectively, ensuring that care coordination runs smoothly.
- Excellent written and oral communication skills, with the ability to interact with providers, patients, and third parties while applying tact and sensitivity.
- Solid understanding of customer service principles, with the ability to navigate patient inquiries with ease.
- Knowledge of available community resources and the ability to evaluate resources and make appropriate referrals.
- Knowledge of health-related financial assistance, including Medicare and Medicaid.
- Ability to interpret and comply with federal, state, and local laws and regulations.
- Ability to deal effectively with a diverse patient population.
- Ability to prepare reports and communicate effectively orally and in writing.
