Trusted Resource Associates (TRA) is seeking a skilled travel nurse to join our team as a Case Management RN. This is a fantastic opportunity for a dedicated healthcare professional to work with a reputable organization and make a meaningful impact in the lives of patients.
Key Responsibilities- Facilitate care along a continuum through effective resource coordination to help patients achieve optimal health, access to care, and appropriate utilization of resources.
- Manage medical necessity process for accurate and timely payment for services, which may require negotiation with a payer on a case-by-case basis.
- Collaborate with healthcare teams to ensure seamless care transitions and optimal patient outcomes.
- Develop and implement individualized care plans that address the unique needs and goals of each patient.
- Monitor and evaluate patient progress, making adjustments to care plans as needed to ensure optimal outcomes.
- Bachelor of Science in Nursing degree
- At least 2 years of acute care experience and 1 year of Case Management experience in an Acute Care Discharge environment
- Alabama State License and BLS certification
- Guaranteed hours for travel contracts
- Preferred booking agreement for local contracts
- Weekly pay
- Holiday pay
- Referral bonus
