About the Role

RADIAS Health is seeking a highly skilled Mental Health Practitioner to join our Community Care Service Team (CCST). As a key member of our team, you will play a vital role in supporting individuals who have been identified as high utilizers of emergency room/hospital services and/or have been identified as someone with high unmet needs.

Key Responsibilities
  • Develop and implement individualized treatment plans in collaboration with clients and the treatment team.
  • Coordinate services with other county or community agencies, including local community hospitals, mental health case management, and outpatient behavioral health providers.
  • Assist clients with all aspects of psychiatric rehabilitation, including independent living skills, transportation, and budgeting.
  • Attend commitment and other court hearings as needed, and complete required court reports.
  • File reports with Vulnerable Adult, Child Protection, and other mandated/required reports when applicable.
Requirements
  • Bachelor's degree in behavioral sciences or related field, or 30 semester credit hours in behavioral health sciences, and 2,000 hours supervised work experience with individuals diagnosed with serious mental illness and/or substance use disorders.
  • Masters or other graduate degree in behavioral health sciences or related field, or 4,000 hours supervised work experience with individuals diagnosed with serious mental illness and/or substance use disorders.
  • Must have a car, auto insurance, and driver's license.
