RADIAS Health is seeking a highly skilled and compassionate Homeless Outreach Mental Health Practitioner to join our team. As a key member of our PATH Access Team, you will provide professional, quality program services to individuals experiencing homelessness, with a focus on those with mental health and/or co-occurring diagnoses.
Key Responsibilities- Provide PATH Services: Offer outreach, screening, case-management, and re-engagement services to PATH eligible/enrolled clients, developing and implementing recovery/housing goals and plans in collaboration with clients.
- Maintain an Active Caseload: Manage a caseload of 15-20 clients, providing regular check-ins and support to ensure their needs are met.
- Collaborate with Multidisciplinary Team: Work closely with an Advanced Nurse Practitioner, Mental Health Professionals, and Certified Peer Specialists to provide comprehensive care.
- Identify and Support Eligible Clients: Identify candidates for Section 811 and PHOP subsidy programs, complete the application process, and facilitate referrals to HSS.
- Target Outreach Efforts: Focus on individuals experiencing homelessness with high acuity and complex needs, ensuring their voices are integrated into PATH program planning and outreach efforts.
- Provide Community Outreach: Engage with people living in places not meant for human habitation, as well as in shelter settings and community drop-in locations.
- Coordinate Cases: Work with RADIAS Health outreach staff and other community outreach providers to ensure seamless service delivery.
- Develop Care Plans: Create and implement individualized care plans with enrolled clients, addressing their unique needs and goals.
- Assist with Daily Living Needs: Help individuals access essential services, such as food, clothing, and healthcare.
- Initial Assessments: Conduct thorough assessments to determine the presence of mental illness, chemical dependency, or behavioral problems.
- Monitor and Referral: Monitor clients who may decline active services, ensuring referral for emergency psychiatric or other crisis intervention services as needed.
- Coordinate Service Provisions: Work with other service providers to ensure smooth, prompt, consistent, and quality delivery of services to clients.
- Refer Urgent Cases: Refer cases where there is risk of harm to self or others to the appropriate hospital emergency room and community crisis teams.
- Provide Information and Referral: Offer information and referral services, as well as intervention strategies and assistance in de-escalating crises.
- Assess Need for Services: Assess the need for hospitalization, crisis residence, or detox services, and provide referrals as necessary.
- Client Advocacy: Provide client advocacy and information and referral services to ensure protection of civil rights and welfare of clients.
- Refer Cases to Adult Protection: Refer cases to Adult Protection and Child Protection where there is evidence of neglect and/or abuse of vulnerable adults and/or children.
- Assist with Legal and Advocacy Services: Assist clients in obtaining needed legal and/or advocacy services.
- File Reports: File all legally required reports with Vulnerable Adults, the Ombudsman, and Child Protection.
- Provide Education: Provide education to clients regarding the symptoms, course, and treatment of their illness.
- Participate in Case Consult Meetings: Participate in regular case consult meetings to ensure comprehensive care.
- Attend Staff Meetings and Trainings: Attend required staff meetings and trainings as scheduled.
- Transport Clients: Transport clients in the community, as needed.
- Complete Paperwork and Charting: Complete all paperwork, reports, and charting contemporaneously and in an organized manner.
- Maintain Regular Work Hours: Maintain a pattern of regular work hours.
- Participate in Professional Growth and Development: Participate actively in on-going professional growth and development, maintaining appropriate professional behavior and participating in appropriate supervision.
- Allow for Flexibility in Hours Worked: Allow for flexibility in hours worked, as needed.
- 6,000 Hours of Supervised Experience: Possess 6,000 hours of supervised experience in the delivery of services to persons with mental illness.
- Experience Supporting Homeless Individuals: Have experience supporting persons experiencing homelessness.
- Valid Driver's License: Possess a current and valid driver's license with current and valid auto insurance and a clean driving record.
- Bachelor's Degree in Behavioral Sciences: Possess a Bachelor's degree in behavioral sciences or related fields.
- Experience Providing Outreach Services: Have experience providing outreach services to persons experiencing homelessness and knowledge of services/resources in Ramsey County.
