Job Title: Licensed Practical Nurse

We are seeking a skilled and compassionate Licensed Practical Nurse (LPN) to join our team at Willowdale Health Services. As a LPN, you will play a vital role in providing high-quality patient care and contributing to the success of our healthcare team.

Job Summary:

Under the supervision of the RN Unit Manager or DON, the LPN will be responsible for providing nursing care to assigned residents/patients, implementing and evaluating their care plans, and supervising the performance of nursing assistants. The ideal candidate will possess excellent communication and interpersonal skills, be able to work effectively in a fast-paced environment, and demonstrate a commitment to delivering exceptional patient care.

Responsibilities:
  • Provide direct patient care, including administering medications, performing treatments, and monitoring vital signs.
  • Develop and implement individualized care plans, taking into account the unique needs and goals of each patient.
  • Supervise and direct the activities of nursing assistants, ensuring that they are providing high-quality care and adhering to established policies and procedures.
  • Collaborate with the healthcare team to ensure seamless communication and coordination of patient care.
  • Participate in ongoing education and training to stay current with best practices and industry developments.
Requirements:
  • Current licensure as an LPN in the state of employment.
  • Must have and maintain current CPR certification.
  • Ability to use necessary supplies and equipment, including feeding pumps, oxygen, treatment trays, and medication carts.
  • Ability to calculate drug dosages and administer medications accurately.
  • Meets all health requirements imposed by law.
  • Ability to relate positively, effectively, and appropriately with patients, families, community members, volunteers, and other center staff.
  • Able to read, write, speak, and understand English, and demonstrates basic computer knowledge and ability with an aptitude to learn company applications.
