Job Title: Licensed Practical Nurse

We are seeking a skilled and compassionate Licensed Practical Nurse (LPN) to join our team at Willowdale Health Services. As a key member of our healthcare team, you will provide high-quality patient care and support to our residents.

Job Summary:

Under the supervision of the RN Unit Manager or DON, the LPN/LVN will perform as a licensed caregiver and assume responsibility and accountability for assigned residents/patients for a shift of duty. Nursing care will be provided through coordination, implementation, and evaluation of the resident's/patient's plan of care.

Responsibilities:
  • Provide direct patient care and support to residents, including administering medications, performing treatments, and providing emotional support.
  • Assess and evaluate resident/patient needs, develop and implement individualized care plans, and coordinate with other healthcare professionals to ensure comprehensive care.
  • Supervise and direct nursing assistants in the delivery of resident/patient care, ensuring that care is provided in a safe and efficient manner.
  • Participate in quality improvement initiatives and contribute to the development of policies and procedures to enhance resident/patient care and services.
Requirements:
  • Current licensure as an LPN in the state of employment.
  • Current CPR certification.
  • Ability to use necessary supplies and equipment, including feeding pumps, oxygen, treatment trays, medication cart, etc.
  • Ability to calculate drug dosage.
  • Meets all health requirements imposed by law.
  • Ability to relate positively, effectively, and appropriately with residents, families, community members, volunteers, and other center staff.
  • Able to read, write, speak, and understand English. Demonstrates basic computer knowledge and ability with an aptitude to learn company applications.
