RN-Manager Ambulatory Care Coordination Specialist

This RN-Manager Ambulatory Care Coordination role is responsible for planning, organizing, and managing the essential functions of the Ambulatory Care Hub in collaboration with the Ambulatory Director of Care Coordination.
The ideal candidate will have three years of RN experience in an acute care setting and leadership experience in an ambulatory care clinic. They will be responsible for overseeing teams driving coordination of care across the patient care continuum, inclusive of care management, support services, patient navigation, and remote patient triage in Ambulatory Services.
The RN-Manager Ambulatory Care Coordination will serve as a liaison among key stakeholders in care coordination program development, accountable for oversight and development of overall care coordination efforts to ensure effective implementation, monitoring, and evaluation.
Key Responsibilities:
- Plans, prepares, implements, monitors, and controls the budget to ensure sound fiscal management consistent with the goals of the hospital.
- Adopts a total performance improvement approach to work that includes employee empowerment, managing with data, a philosophy of continuous improvement, a customer-oriented attitude, and a work methodology that minimizes error.
- Ensures program activities are coordinated, aligned, and standardized across the care continuum.
- Reports performance improvement results and addresses any problems or issues accurately and timely.
- Participates in the development of tactical plans for the area(s) of responsibility.
- Implements policies, procedures, and directives as appropriate.
- Uses system thinking concepts to determine opportunities for improvement.
- Ensures a high level of employee, patient, and physician satisfaction, and maintains positive relations with direct and indirect customers.
- Establishes program goals that are consistent with organizational goals.
