About the Role

We are seeking a skilled Physical Therapist Assistant to join our team at HealthMark Rehabilitation. As a key member of our interdisciplinary team, you will play a vital role in providing high-quality care to our patients in a geriatric rehabilitation setting.

Responsibilities
  • Provide Skilled Therapy Services: Evaluate and treat residents and short-term patients in accordance with physician's orders and accepted professional practices.
  • Develop Comprehensive Therapy Plans: Create overall therapy plans of care, including treatment and medical diagnosis, planned duration and frequency, and short and long-term goals that support the discharge plan.
  • Document Patient Progress: Document observations, patient's response to skilled interventions/treatment programs, degree of progress, and notable changes in patient's condition within specific timeframes.
  • Meet Regulatory Requirements: Complete all documentation required timely and in accordance with state and federal regulations and policies/guidelines.
  • Assist with Facility Maintenance: Assist with maintaining the rehab gym equipment and treatment area in a safe manner, using good infection prevention methods.
Requirements
  • License/Certification: Must hold Texas license/certification for Physical Therapy Assistant.
  • Communication Skills: Possess strong oral and written communication skills, organizational and analytical skills, and ability to function effectively in multi-faceted systems.
  • Physical Requirements: Meet physical requirements to deliver responsibilities.
  • Experience: Experience in a geriatric rehabilitation setting preferred.
  • Background Check: Successful completion of hiring process and background check.
