Job Summary:
We are seeking an experienced Registered Nurse (RN) to join our Hospice Care Team. As a key member of our interdisciplinary team, you will provide end-of-life care to patients and their families, working closely with physicians, social workers, and other healthcare professionals to ensure seamless coordination of care.
Responsibilities:
• Assess and coordinate patient discharge planning needs with the healthcare team
• Perform point-of-care testing according to established policies and procedures
• Collaborate with physicians regarding plan of care and changes to patient status, providing regular updates and recommending revisions as necessary
• Coordinate communication with multidisciplinary team members and outside agencies to achieve expected outcomes for patient care and education through end-of-life or discharge
Requirements:
• Graduate of an accredited nursing program or equivalent educational background for licensure
• Valid RN license in Illinois, Missouri, Oklahoma, or Wisconsin
• Basic Life Support (BLS) certification from the American Heart Association
• Strong communication and interpersonal skills, with ability to work effectively in a fast-paced environment
