Job Summary

We are seeking a highly skilled Registered Nurse Case Manager to join our team at Trinity Health. As a key member of our healthcare team, you will be responsible for coordinating patient care and ensuring seamless transitions between healthcare settings.

Key Responsibilities
  • Coordinate Patient Care: Work with patients, families, and healthcare providers to develop and implement individualized care plans that meet the unique needs of each patient.
  • Assess Patient Needs: Conduct thorough assessments of patients' medical, social, and emotional needs to identify areas for improvement and develop strategies for optimal care.
  • Communicate Effectively: Collaborate with healthcare teams, patients, and families to ensure clear and effective communication, promoting a culture of transparency and respect.
  • Manage Patient Transitions: Coordinate patient transfers between healthcare settings, ensuring a smooth transition and minimizing the risk of readmission.
  • Advocate for Patients: Act as a patient advocate, promoting their rights, dignity, and well-being throughout the healthcare journey.
Requirements
  • Education: Associate degree from an accredited RN program.
  • Licensure: Current license to practice as a registered nurse in the State of Ohio.
  • Experience: Minimum of 3-5 years recent clinical experience in an acute care setting.
