We are seeking a highly skilled Physical Therapist to join our home healthcare team at Western CT Home Care Inc. As a Physical Therapist with us, you will have the opportunity to work independently at the top of your professional license while being part of a coordinated home health team.

About the Role

We are looking for a qualified Physical Therapist to provide services that identify, prevent, remediate and rehabilitate acute or prolonged physical dysfunction or pain. The ideal candidate will have experience in qualified home health agency and be able to perform physical therapy assessment including functional limitations, rooms, capabilities, mobility status, endurance status, ADL status.

Responsibilities
  • Perform physical therapy assessment including functional limitations, rooms, capabilities, mobility status, endurance status, ADL status.
  • Develop and implement individualized plans of care in partnership with the patient, representative (if any), and caregiver.
  • Communicate with the physician(s) involved in the plan of care and other health care providers as appropriate.
Requirements

The ideal candidate will have:

  • Bachelor's degree in Physical Therapy.
  • Current Physical Therapist license.
  • Minimum one (1) year of experience.

Salary Estimate

The estimated salary for this role is $65-$75 per hour, based on the location and industry standards.
