Our non-acute care services are supported by various affiliates, ensuring that patients receive the highest quality of rehabilitation and skilled nursing services.


Position Overview:

Under the guidance of the rehabilitation manager or supervisor, the therapist conducts assessments, formulates treatment strategies, and implements interventions in alignment with professional standards, state regulations, and organizational policies. This role encompasses a diverse patient demographic, from infants to the elderly, within an outpatient care framework.

Opportunities for rotation to acute care settings may be available based on operational requirements.

Key Responsibilities:

  • Conducts therapy assessments, designs treatment plans, and sets treatment objectives with patient collaboration, utilizing evidence-based practices and APTA guidelines.

Provides specialized care for:
Oncology, Lymphedema, Amputee, Spinal Cord Injury, Stroke, Total Joint Replacement, Orthopedic and Neurological conditions. Oversees and delegates patient-related tasks to assistants and support staff. Mentors students as assigned. Collaborates with interdisciplinary teams to ensure cohesive patient care. Effectively manages time and demonstrates flexibility in scheduling within designated hours.

Independently makes decisions within the defined scope of practice while seeking guidance when necessary.

Meets or surpasses productivity and documentation standards. Welcomes and utilizes constructive feedback, maintaining respectful communication with colleagues. Assumes additional responsibilities as assigned and adheres to all departmental and organizational policies. Exhibits adaptability and a willingness to engage in new initiatives. Participates in departmental quality improvement efforts as assigned. Assists in prioritizing patient care effectively. Contributes to peer review processes as required. Supports the organization’s commitment to excellence and patient satisfaction.

Experience in delivering comprehensive outpatient care to patients with physical impairments resulting from various conditions is essential.

Familiarity with a range of diagnoses, including low complexity medical cases, orthopedic, oncology, and neurological conditions, is preferred.

Qualifications:

Must hold a current license to practice in the relevant state, having successfully completed all educational and clinical requirements for licensure.

Temporary licensure may be considered at the discretion of management.

CPR/AED certification is required.
