  • Collaborate with healthcare teams to ensure seamless care coordination and communication.
  • Provide education and support to patients and their families to promote self-care and independence.
  • Document patient progress and outcomes in a timely and accurate manner.
Requirements:
  • Current Registered Nurse license in the State of New Jersey.
  • At least 1 year of experience in hospice, home care, and/or community health.
  • Strong communication and interpersonal skills.
  • Ability to work independently and as part of a team.
  • Excellent problem-solving and critical thinking skills.
