Compensation: $32.38/hr, DOE

Employment Type: Full-Time, 40 hours/week

Availability: Monday to Friday

Key Responsibilities:
  1. Conduct patient triage, including vital signs, health history, and medication reconciliation.
  2. Document patient data in the Electronic Health Record, adhering to established standards.
  3. Provide educational support to patients and families, focusing on self-care tailored to their needs.
  4. Administer medications as per provider directives.
  5. Prepare examination rooms with necessary equipment and supplies.
  6. Assist providers during examinations and procedures as requested.
  7. Perform lab tests, maintain logs, and report critical lab values promptly.
  8. Administer vaccines according to current schedules.
  9. Engage in team huddles with primary care staff.
  10. Manage telephone encounters and medication refill requests.
  11. Utilize electronic medical records for patient visit documentation.
  12. Keep client medical records current for audits and reviews.
  13. Participate in quality assurance and improvement initiatives.
  14. Collaborate effectively with primary care teams.
  15. Communicate clearly and professionally with patients and team members.
  16. Contact patients regarding lab results.
  17. Identify patients for specialized treatment services.
  18. Assess health literacy needs and provide appropriate educational materials.
  19. Conduct medication reviews upon request.
  20. Develop or source suitable patient education materials.
  21. Assist with outreach activities, including health fairs.
  22. Work collaboratively to meet accreditation and regulatory requirements.
  23. Adhere to all One Health policies and procedures.
  24. Engage in performance optimization and risk management activities.
  25. Comply with all elements of One Health's Compliance Program.
  26. Deliver exceptional customer service, reflecting One Health's mission.
  27. Perform additional duties as assigned.
Minimum Qualifications:

Education/Certificates/Licenses:

  • Nursing degree from an accredited program
  • Current state nursing license
  • BLS Certification upon hire
  • Valid driver's license

Experience:

  • Preferred: 2 to 5 years of experience in a primary care clinical setting
