End-of-Life Care Specialist

We are seeking an experienced Nurse Practitioner - Hospice to join our team at Haven Hospice. As a valued member of our organization, you will play a critical role in delivering compassionate and high-quality care to patients and their families.
About the RoleThis is a full-time opportunity for a Nurse Practitioner with a passion for end-of-life care. You will be responsible for evaluating, assessing, and diagnosing medical conditions, developing plans of care, and prescribing medication and treatment. Your expertise and compassion will make a significant difference in the lives of our patients and their loved ones.
Key Responsibilities- Evaluating patient needs and creating personalized care plans to ensure comfort, dignity, and quality of life;
- Collaborating with healthcare professionals to coordinate care and address complex medical issues;
- Providing emotional support and education to patients and their families to empower them to make informed decisions about their care;
- Maintaining accurate and detailed clinical notes to ensure seamless communication among team members;
- Participating in interdisciplinary meetings to share knowledge and best practices with colleagues;
- Staying up-to-date on relevant laws, regulations, and industry standards to ensure compliance.
- A Master's degree from an accredited Nurse Practitioner program;
- A minimum of one year of experience as a Nurse Practitioner, preferably in hospice or palliative care;
- A current DEA license (if required by state regulations);
- A valid nursing license in the state of practice;
- CPR certification;
- Certification in a specialist area (e.g., CHPN or Geriatrics) preferred;
- Commitment to clinical excellence, documentation, and continuous learning;
- Ability to work collaboratively as part of a multidisciplinary team.
