We are seeking a skilled Registered Nurse to join our team at LCH Health and Community Services. As a key member of our interdisciplinary team, you will provide high-quality chronic care management to our patients, perform triage, and assist in the care of primary care patients, Women's Health, and Pediatric patients.
Key Responsibilities- Provide patient-centered comprehensive healthcare through motivational interviewing, healthcare coaching, chronic disease management, and population health management.
- Supervise and manage unlicensed staff to ensure seamless care delivery.
- Collaborate with our team to provide team-based care, including Providers, Medical Assistants, Patient Service Representatives, and ancillary services.
- Bilingual proficiency in English and Spanish required.
- Graduate of an accredited School of Nursing with a bachelor's degree in Nursing.
- Holds current, or eligible for, RN license in the Commonwealth of PA.
- Holds current CPR certification.
- Medical/Dental/Vision/Prescription insurance.
- Employer-paid basic life/LTD insurance.
