As a Social Worker II, you will be responsible for conducting psychosocial screenings and assessments, developing and implementing individualized care plans, and providing emotional support and guidance to patients and their loved ones.

We are looking for a skilled and compassionate professional who is able to communicate effectively with patients, families, and healthcare teams, and who is committed to delivering patient-centered care.

Key Responsibilities:

  • Conduct psychosocial screenings and assessments to identify patient needs and develop individualized care plans.
  • Provide emotional support and guidance to patients and their families, addressing their psychosocial needs and concerns.
  • Collaborate with healthcare teams to ensure comprehensive and coordinated care.
  • Develop and maintain relationships with community agencies and resources to support patient needs.

Requirements:

  • Master's Degree in Social Work from an accredited institution.
  • Licensed by the Texas State Board of Social Work Examiners.
  • Excellent communication and interpersonal skills.
  • Ability to work effectively in a fast-paced healthcare environment.
