Job Title: Social Adjustment Services Case Specialist

We are seeking a highly motivated and compassionate individual to join our team as a Social Adjustment Services Case Specialist. As a key member of our team, you will play a vital role in helping clients achieve self-sufficiency and social integration through our Social Adjustment Services Program.

Job Summary:

The Social Adjustment Services Case Specialist will be responsible for completing intake/enrollment for eligible clients, assessing client needs, and creating a plan for the client to reach self-sufficiency and social integration. You will also assist clients in developing life skills and navigating American systems, enter client and service information in our database, and maintain detailed case files.

Key Responsibilities:
  • Complete intake/enrollment for eligible clients
  • Assess client needs and create a plan for self-sufficiency and social integration
  • Assist clients in developing life skills and navigating American systems
  • Enter client and service information in our database
  • Maintain detailed case files
  • Provide transportation to initial doctor appointments, benefit interviews, and trainings for clients as needed
  • Assist clients in scheduling transportation benefits as provided by RMA and Medicaid
  • Provide interpretation for SAS related services as needed
  • Assist clients with application forms for assistance, such as government housing and Food Stamp/Medicaid
  • Follow up with Food Stamp/Medicaid office and SSA regarding client's application until decision is made
  • Equip clients to conduct independent services
  • Refer clients to other programs and services as needed
  • Evaluate and monitor quality of data entry by other agency staff to ensure data integrity for program requirements
Requirements:
  • Mature and personal Christian faith
  • Committed to the mission, vision, and values of World Relief
  • Desire to serve and empower the Church to impact vulnerable communities
  • Able to affirm and/or acknowledge World Relief's Core Beliefs, Statement of Faith, Christian Identity, and National Evangelicals For the Health of The Nation document
  • Proficient in Microsoft Office applications
  • Valid Driver's License and good driving record
  • Ability to maintain records and reports
  • Detail-oriented and organized
  • Demonstrates initiative, flexibility, and responsibility
  • Team player and collaborator
  • Previous experience as a Case Manager
Preferred Qualifications:
  • Fluency in an additional language such as Spanish, Arabic, Farsi, Dari, Pashto, Haitian Creole, Swahili, and Pashto is preferred
  • Cross-cultural experience strongly preferred
  • Ability to manage multiple demands and deadlines with flexibility
  • Ability to communicate clearly, both orally and in writing
  • Effectively manage stressful or crisis situations
