Key Responsibilities:

  • Provide personalized care to patients to improve their quality of life.
  • Offer mental, emotional, and physical support to patients and their families as they navigate the end-of-life care process.
  • Conduct initial and ongoing comprehensive assessments of patients.
  • Obtain medical histories from patients and/or family members.
  • Conduct physical examinations, including vital signs, physical assessments, mental status, appetite, and diet.
  • Evaluate patients, family members, and home situations to determine required health teaching.
  • Evaluate patients' environments to determine available assistance from family members and necessary services.
  • Develop and implement initial plans of care and work with other members of the Interdisciplinary Group (IDG) to complete and maintain comprehensive plans of care.
  • Track patients' symptoms, conditions, and behaviors to identify changes or signs of physical and mental decline.
  • Consult with attending physicians and Hospice Medical Directors concerning changes in patients' conditions requiring order changes and/or changes in plans of care.
  • Coordinate patient services with other members of the IDG and contracted services when needed.
  • Discuss with supervisors if IDG determines that patients would benefit from contracted services.
  • Obtain physicians' orders for supplies, including hospital beds, oxygen, and medications for patients.
  • Educate patients' family members about disease progression, medication, nutrition, and hospice philosophy.
  • Collaborate with specialists to create care plans and understand individuals' distinct conditions and requirements.
  • Evaluate the effectiveness of hospice services to patients and families on an ongoing basis.
  • Perform on-call duties (including holidays) per policy and business needs.
  • Actively participate in Quality Assurance and Performance Improvement (QAPI) meetings and activities.
  • Adhere to Federal, state, and accreditation requirements, including Medicare and Medicaid regulations.

Management Responsibilities:

  • Perform administrative activities necessary for the effective management of the department.
  • Provide input for the selection and development of employees.
  • Employee counseling and motivation.
  • Organization of goals and objectives.
  • Planning, organizing, integrating, and measuring the work performed within the department.
  • Delegate responsibilities to staff with clear expectations.

Education and Experience:

  • Completion of an accredited RN program.
  • Clinical background from three to five years with experience in geriatrics; long-term care preferred.
  • Proficient assessment skills related to geriatric residents.
  • Fosters an environment honoring resident/patient preferences and choices.
  • Exhibits well-developed communication skills.
  • Ability to cope with mental and emotional stress related to the position.
  • Function independently and have flexibility and professionalism.
  • Very strong interpersonal skills and the ability to build relationships with staff, leaders, vendors, and other stakeholders.
  • Expert level written and verbal communication skills.
  • Demonstrated proactive approaches to problem-solving with strong decision-making capability.
  • Demonstrate the highest level of customer/client service and response.
  • Work collaboratively in a team environment that promotes inclusiveness and communication among team members.

Certificates, Licenses, Registrations:

  • Licensed as a registered nurse in the state in which employed.
  • Current Basic Life Support (BLS) certification or ability to obtain prior to direct patient contact.
  • Valid driver's license and proof of insurance required.
