We are seeking a skilled Registered Nurse to join our team as a Case Manager. In this role, you will be responsible for planning, organizing, and directing hospice care for patients and their families.
Key Responsibilities- Conduct comprehensive assessments of patients and families to determine their needs and develop individualized care plans.
- Provide professional nursing care, including administering medications, treatments, and counseling patients and their families.
- Coordinate patient care with the interdisciplinary team, including physicians, social workers, and other healthcare professionals.
- Communicate effectively with patients, families, and healthcare team members to ensure seamless care delivery.
- Participate in quality improvement initiatives and contribute to the development of policies and procedures.
- Graduate of an accredited school of nursing.
- One to two years of recent acute care experience in an institutional setting, and one to two years of recent experience in home care preferred.
- Current licensure in good standing.
- Excellent observation, verbal and written communication skills, problem-solving skills, basic math skills, and nursing skills per competency checklist.
