Job Summary

Agrace is seeking a skilled Registered Nurse Case Manager to join our Homecare team. As a key member of our interdisciplinary team, you will be responsible for providing high-quality patient care and case management services to patients and their families in their homes.

Key Responsibilities
  • Conduct comprehensive assessments of patients' physical, emotional, and social needs
  • Develop and implement individualized plans of care to meet patients' needs
  • Collaborate with patients, families, and healthcare providers to ensure seamless care coordination
  • Provide education and support to patients and families to promote self-care and independence
  • Document patient information and outcomes in electronic health records
Requirements
  • Current, unrestricted Registered Nursing License in the state of employment
  • One year of nursing experience, preferably in hospice or homecare
  • Ability to develop and maintain knowledge of hospice and end-of-life care
  • Excellent communication and interpersonal skills
  • Ability to work independently and as part of a team
